Authorization. I authorize Advance Restoration
Inc. to inspect the property and perform all emergency mitigation
work reasonably necessary to protect the property from further
damage. This may include, but is not limited to: water extraction,
structural drying, demolition of affected materials, sewage
cleanup, mold prevention, antimicrobial application, HEPA
vacuuming, air filtration, temporary board-up, temporary plumbing
or electrical protection, contents manipulation, moisture
monitoring, and daily equipment inspections.
Insurance & Payment. I authorize Advance
Restoration Inc. to communicate directly with my insurance company
regarding this claim. I understand that insurance coverage is
determined solely by my insurance policy. I remain responsible for
all charges not covered by insurance, including deductibles,
exclusions, policy limits, depreciation, or denied portions of the
claim.
Direction to Pay. I authorize my insurer, where
permitted, to issue payment directly to Advance Restoration Inc.
for services performed.
Removal of Materials. I authorize the removal and
disposal of damaged materials required to complete emergency
mitigation and reduce further damage.
Equipment Responsibility. I agree to keep all
drying equipment plugged in and operating continuously unless
instructed otherwise. I will not move or tamper with equipment and
will notify Advance Restoration Inc. immediately if equipment
stops operating.
Documentation. I authorize photographs, moisture
readings, sketches, and other documentation to be collected and
shared with my insurance company for claim purposes.
Acknowledgement. I understand that hidden damage
may be discovered during mitigation and that additional work may
become necessary. Emergency mitigation is intended to reduce
further damage but cannot guarantee the prevention of all
secondary damage.
